4 STEPS TO SCHEDULE YOUR APPOINTMENT:
#1. CALL US
The first step is to call our office. Have your insurance card ready. We will set up your appointment. Payment will be discussed at this time but won't be due until the time of service.
Scheduling is based on your availability and the therapist availability. We try our best to work with your schedule.
#2. Phone screening
You will be asked to share why you are seeking counseling services. We work with most individuals seeking services, however, we do not service those suffering from chronic mental illness, chronic substance abuse or have a history of violence. We are happy to refer those individuals to appropriate service providers. We ask that you be honest in your answers to that we may place you with the therapist that can best meet your needs.
#3. a bit of paperwork
Prior to your first appointment you will be asked to complete a detailed background questionnaire, as completely as possible, any information left out could slow the process of treatment. This allows your therapist to better understand your situation. We have a link that we can send you so that you may take your time filling it out, when you are completed and submit the document, we receive it back electronically and it is uploaded in your file for the therapist.
Our system is 100% confidential and follows HIPAA Compliance.
